Assist Access/Maintain Employment

Assist Personal Activities
My Premium Care supports you in performing daily activities and household tasks.
If you are an NDIS participant who has been approved for the Core Supports category, then you can access these services through your NDIS plan. Household tasks
Our trained support workers will be by your side to assist with all types of essential domestic chores at whichever level you require. This kind of support is designed to facilitate your daily independence, while also helping you to build the skills needed to carry out a range of household activities – all provided in the comfort of your own home.

Assist-Travel/Transport
Transport supports are given to those participants who find difficulty travelling on their own without any assistance. The team at My Premium Care provides complete transport support to participants to ensure that they have the capacity to travel independently and safely with the right equipment and travel assistance. Transport supports mostly include personal transport-related aids and equipment. We also train participants to use public transport so that they can travel freely.
For those participants who are not able to travel independently, their transport support costs can include the reasonable and necessary costs of taxis or other private transport options.
Transport supports are only applicable to participants and cannot be applied to travel for families, carers, or providers of support.
Development of Daily Living & Life Skills
Development of daily living and life skills focuses on training and development activities undertaken by the participant or their carer to increase their ability to live as autonomously as possible. The supports provided will enhance the ability of the participant to travel, use public transport independently, practice and maintain personal hygiene regularly and many more. Life Skills are abilities for adaptive and positive behaviour that enable humans to deal effectively with the demands and challenges of life. My premium Care will assist you and your families or carers to overcome challenges yo may encounter in your daily life assisting you to implement strategies that will help you manage these challenges.
My Premium Care provides training and development of life skills, including personal hygiene, personal care, preparing a healthy meal, maintaining cleanliness in-home, toileting, laundering of clothes, communicating with service providers and support networks, transport and travel, accessing public transport and many more according to your needs.
Assist Daily Tasks/Shared Living
The Supported Independent Living (SIL) service includes offering support in terms of help in prepping meals and cleaning, assistance in personal care tasks, support during medical appointments, and so on. In an SIL service, the assistant or the support staff either stays with the person or in a nearby building within quick access and attends to the person when required. This way, the participant does not feel that they are fully dependent on another individual.
It is important to understand that Supported Independent Living is a little different to Specialist Disability Accommodation, SIL encourages participants to have more choice and control.
Members can have access to Supported Independent Living accommodation if they have a private space in case they are living at home with their families, living alone, or sharing with others. Our NDIS planning manager will help you find out if your accommodation falls under the SIL category.
Assist-Life Stages, Transition, and Support
Assistance in coordinating or managing life stages, transition, and support includes short and long-term support that focuses on strengthening the participant’s ability to coordinate their support, and to assist them to live at home and participate in their community. Includes:
- support connection
- coordination of supports
- assistance with accommodation and tenancy obligations
- life transition planning including mentoring, peer support, and individual skill development
- assistance with decision making, daily planning, budgeting
Participation in Community, Social & Civic Activities
- This funding covers the support NDIS participants need to meet goals relating to participating in community, social or recreational activities. One example is funding for a Disability Support Worker to support you to travel to and attend a music concert. This funding also covers support for activities that are either centre-based (e.g. to attend a Community Hub) or out in the community (e.g. to attend a book group).
- Under Assistance with Social and Community Participation, the NDIS may fund a support worker to support you to:
- attend personal development courses
- join a social group
- take an outing or a holiday
- go on a holiday camp
- visit your local library
- see a movie or a concert
- go to community events, and/or
- join a sporting club
Group/Centre-Based Activities

Short Term Accommodation
Short Term Accommodation, including respite, is support for when you need to live out of home for a short period.
Short Term Accommodation funding can be used for respite to support you and your carers. This gives your carers a short break from their caring role. Sometimes a short stay away from home:

- can be a place to make new friends or develop new skills
- may help to maintain your current living situation by giving your informal support a break.
