Support Coordination
A Premium Care will work with you to ensure you have and build the skills you need to understand and implement the use of your plan. Our experienced support coordinators will work with you to ensure a mix of supports is used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently, have a voice, and be included in your community. It is your choice and we are here to help you along the way.
  • Support connection – This support is to build your ability to connect with informal, community, and funded supports enabling you to get the most out of your plan and pursue your goals.
  • Support coordination – coordination of supports: This support will assist you to build the skills you need to understand, implement and use your plan. A support coordinator will work with you to ensure a mix of supports is used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in your community.
We respect your choice and control.
Three Levels of Support Coordination:
  • Support Connection: Think of it as your link to informal, community, and funded supports. It’s all about helping you make the most of your plan and reaching your goals.
  • Support Coordination (Coordination of Supports): This level is all about giving you the skills to understand and use your plan effectively. Your support coordinator will work closely with you to make sure you have the right mix of supports to maintain relationships, handle service tasks, live more independently, and be part of your community.
  • Specialist Support Coordination: This is like the advanced support coordination. It’s for people dealing with complex situations who need specialized help. A specialist Support Coordinator will assist you in managing challenges in your support system and make sure you get consistent service. Your needs come first.

Assist Access/Maintain Employment

If you’re considering a transition while in supported employment, it might be helpful to discuss your new work objectives with your LAC or Support Coordinator.

Our training and assessment program is crafted to enhance skills, catering to your specific needs or those of your loved one. Assist Travel/Transport

At A Premium Care, our mission is to assist participants who encounter difficulties traveling alone by offering transport support services. We prioritise safe and independent travel experiences, providing comprehensive aid and essential tools for your journey. This encompasses personal aids and transportation-related equipment, ensuring that you have the necessary resources.

In addition to equipment provisions, we offer training sessions that focus on navigating public transportation systems, empowering individuals to travel confidently on their own.

For those who require additional assistance and cannot travel independently, our transport support includes covering reasonable expenses for taxis or other private transportation alternatives.

Please note that our transport support services are specifically designated for participants and cannot be utilized to cover travel expenses for family members, caregivers, or support providers.

Development of Daily Living & Life Skills

The emphasis on developing daily living and life skills is to empower participants or their caregivers towards fostering greater independence. These tailored supports focus on enhancing a range of capabilities, including, but not limited to, travel, utilising public transport independently, maintaining consistent personal hygiene, and other vital skills essential for daily living.

At A Premium Care, our commitment is to providing personalised training and development in life skills tailored to meet your specific needs. These encompass a range of areas such as personal hygiene, self-care practices, meal preparation, household upkeep, laundry, effective communication with service providers and support networks, managing transport needs, accessing public transportation, and more.

Our goal is to assist you and your families or caregivers in navigating and conquering daily challenges by implementing practical and efficient strategies.

Support Independent Living

The Supported Independent Living (SIL) service offers comprehensive support, including meal preparation, cleaning, personal care assistance, and aid during medical appointments. In SIL, the support staff remains either with the individual or in proximity, providing prompt assistance when needed. This approach aims to maintain the participant’s independence without a sense of complete reliance on another person.

It’s crucial to distinguish SIL from Specialist Disability Accommodation, as SIL promotes participant autonomy and decision-making. Individuals may access SIL accommodation whether they live with their families, independently or in shared arrangements, provided they have their own space.

Our NDIS planning manager can assess if your accommodation qualifies under SIL guidelines.

Assist-Life Stages, Transition, & Support

Our comprehensive life stage and transition support offer both short and long-term assistance. It’s designed to empower participants by enhancing their coordination of support, enabling them to live independently at home and actively engage in their community.

This service encompasses several elements:

  • Support connection

  • Assistance with accommodation and tenancy obligations
  • Life transition planning, including mentoring, peer support, and individual skill development

Participation In Community, Social & Civic Activities

This funding is aimed at supporting NDIS participants in achieving their goals related to community, social, or recreational activities. For instance, it could cover the cost of a Disability Support Worker helping you travel to and attend a music concert. It also extends to supporting both centre-based activities, such as attending a Community Hub, and community-based activities, like participating in a book group.

Under the umbrella of Assistance with Social and Community Participation, the NDIS may provide funding for a support worker to assist you in:

  • Attending personal development courses
  • Joining social groups
  • Going on outings or holidays
  • Participating in holiday camps
  • Visiting local libraries
  • Watching movies or attending concerts
  • Engaging in community events
  • Joining sporting clubs
